Medical/Biohazardous Waste Disposal Form

Skidmore College contracts with a professional waste company for all medical/biohazardous waste disposal. Facilities Services is responsible for coordination of biohazardous waste disposal. When you have generated biohazardous waste, you may request participation in the next regular scheduled biohazardous waste pick-up with this forms.

After completing the appropriate form, click on the Submit button or fax it to attention Facilities Services 580-5880. You will then be contacted as to when to expect pick-up.
